Terms of service and e-Registration

§ 1. General provisions

  1. These regulations define the rules for online booking of physiotherapy services and recreational services (gym, sauna, personal training) provided by Cuprum-Med Sp. z o.o., NIP: 6922506490.
  2. The reservation and sales system is operated by the Caldis.pl platform.
  3. Physiotherapy services are health services within the meaning of the Polish Act on Medical Activity.

§ 2. Payment processing (Przelewy24 operator)

  1. Online payments are handled by Przelewy24 service, operated by PayPro S.A., Poznań (60-198), Pastelowa 8.
  2. A reservation is confirmed only after Przelewy24 returns the transaction status: "Completed".
  3. Available payment methods: fast electronic transfers, payment cards, BLIK.
  4. The Administrator charges no additional fees for online payments.

§ 3. Complaints regarding payments and services

  1. Technical complaints (payment): Issues related to the transaction (e.g. bank connection error) should be reported directly to the operator: serwis@przelewy24.pl.
  2. Substantive complaints (service): Complaints about service quality should be sent to: kontakt@cuprummed.pl.
  3. The complaint should include: full name, date of service and description of the issue.
  4. The Administrator processes complaints within 14 days and informs the Client electronically.

§ 4. Returns and cancellation (24-hour rule)

  1. The Client/Patient has the right to cancel the service without giving a reason.
  2. 24-hour rule: Full refund is available only if the reservation is cancelled at least 24 hours before the scheduled time.
  3. In case of cancellation later than 24 hours or no-show, funds are non-refundable. They constitute lump-sum reimbursement for personnel readiness.
  4. Refunds are processed automatically or manually via Przelewy24.
  5. Money is refunded using the same payment method.
  6. Refund processing time: typically 3–7 business days.

§ 5. Service usage rules

  1. Physiotherapy: The patient must inform the physiotherapist about their health condition and contraindications.
  2. Gym and training: The Client declares no health contraindications. Change of footwear is required.
  3. Sauna: Use at your own risk. Use by persons under the influence of alcohol is prohibited.

§ 6. Data protection (GDPR)

  1. The data controller is Cuprum-Med Sp. z o.o. Data is protected per medical records standards.
  2. Caldis.pl and PayPro S.A. act as data processors.
  3. Transaction data is processed exclusively in the secure Przelewy24 environment.
